Association for Paediatric Palliative Medicine Formulary 2024 (6th edition)
This latest edition represents a substantive revision from the previous version.
The entire formulary has been reviewed and updated incorporating recent published literature, specialist advice and feedback from formulary users. Many of the monographs have been extensively rewritten and references have been brought up to date. The formulary has also been completely reformatted, including greater use of tables and with the aim of improving clarity and navigability.

Important changes include:
Substantially revised or reformatted opioid equivalence tables including new recommended approximate equi-analgesic ratios for morphine, diamorphine and oxycodone
Use of a QR code to link the printed formulary directly to the APPM Website for updates and supplementary information

Association for Paediatric Palliative Medicine - Formulary 2024 (6th edition) Formulary Supplement - prescribing in Chronic Kidney Disease 1st Edition 2025 We are delighted to announce that the APPM formulary supplement “Prescribing in Chronic Kidney Disease” is now available to download from the APPM website The APPM formulary supplement “Prescribing in Chronic Kidney Disease” provides extra support for managing chronic kidney disease in a palliative care context. Each of the 100+ drugs in the Formulary has been rated by kidney function impact: Green (no adjustment), Amber (adjustment needed), and Red (generally not recommended). The guidance should be used alongside the main formulary, considering polypharmacy and starting doses cautiously. Sections are organized by symptom for easier identification of safer alternatives. As ever, we welcome feedback on this new resource as well as feedback on the formulary itself.
Translated versions of the APPM Master Formulary
The APPM formulary was previously translated into Greek, Ukranian and Russian. Use of out of date editions of the Formulary is generally not recommended as dosing recommendations change.
A German Translation of the APPM Formulary 6th Edition 2024 is in development
Work is also beginning on an international version of the APPM Formulary
